* indicates required field.

Your Information
Name

Mailing Address
Your Request
Provide a brief and clear description of the information you'd like to receive. Each request should describe the particular record to the fullest extent possible. The request should describe the subject matter of the record, and if known, indicate where it was created, when it was created, and the person or office that created the record. Please be as specific as possible.
Fees
Please select the category that best fits you. This information helps FHWA FOIA personnel determine what fees, if any, may be applicable. This is required pursuant to 49 C.F.R. § 7.24(b).
Enter the maximum dollar amount you are willing to pay for the request. You will be notified if your fees exceed $25 or the maximum amount you are willing to pay.
Select "No" if you are not requesting a fee waiver. If "Yes", you must explain how your request meets waiver requirements.
FHWA will grant a fee waiver when disclosure of the requested information is in the public interest because it is likely to contribute significantly to public understanding of the operations and activities of the government and is not primarily in the commercial interest of the requester. If applicable, explain how your request meets this requirement.
Expedited Processing
Select "No" if you are not requesting expedited processing. If "Yes", you must demonstrate a “compelling need” for the records.
Provide a detailed explanation for how your request meets the expedition standards described above.
